SACRAMENTS:
If your child is planning on celebrating the Sacraments of Reconciliation and First Holy Communion, he/she needs to at least be entering 2nd grade or 3rd grade and be involved in Faith Formation for two consecutive years (Sac Prep 1 and Sac Prep 2 classes).
If your child is in the 3rd grade (or up), and this is the first year of faith formation, or it has been more than a year since active in a faith formation program, AND child needs to prepare for Sacraments, he/she will be in a class called "TIPS" (Together In Preparation for Sacraments,) which is an OCIA class adapted for children. This class meets every week, along with at least one parent in attendance.

PLEASE NOTE: 
If your child prepares for and celebrates the Sacraments at St Thomas Aquinas, you must be a registered parishioner. Click here to register as a parishioner. 

Also, we must receive your child's Baptismal Certificate no later than Nov. 1 if they are preparing for First Holy Communion, even if they were baptized at STA. You may request your copy from James Taylor in the parish office at taylor@stadallas.org.  Please scan all Baptismal certificates to antosh@stadallas.org. If your child was baptized in another church, please contact them asap to get your copy if you don't have one on hand.

On behalf of myself and/or my child, I specifically waive all rights to privacy and confidentiality with respect to name, likeness, voice, photographs, images, video recordings, audio recordings and identifying information.
I hereby release and agree to fully and unconditionally protect, indemnify, and defend Parish, the Roman Catholic Diocese of Dallas, and their respective officers, agents, and employees, (collectively, “Indemnitees”) and hold each Indemnitee harmless from and against any and all costs, expenses, attorney’s fees, claims damages, demands, suits, judgments, losses, or liability for injuries to property, injuries to persons (including Child) and from any other costs, expenses, attorney fees, claims, suits judgments, losses, or liabilities of any and every nature whatsoever arising in any manner, directly or indirectly, out of, in connection with, in the course of, or incidental to the use or publication of any photographs, videos, or other images of Child, REGARDLESS OF CAUSE OR OF THE JOINT, COMPARATIVE, OR CONCURRENT NEGLIGENCE OF THE INDEMNITEES.
