For good and valuable consideration, I hereby grant to St Thomas Aquinas (“Parish”) the irrevocable and unrestricted right to make, use and/or publish any and all photographs, videos, and other images of me/my minor child (“Child”), or images in which Child may be included, now existing or hereafter made, in any case, with or without identifying Child for editorial, advertising, news, or any other purpose and in any manner and medium; to alter the same without restriction; and to copyright the same. On behalf of myself and/or my child, I specifically waive all rights to privacy and confidentiality with respect to name, likeness, voice, photographs, images, video recordings, audio recordings and identifying information.

Financial support collected solely supports the ministry and our youth. Your financial support offsets the salaries, curriculum, and other materials and resources used throughout the year. The parish subsidizes over half of the cost for each child from weekly parishioner contributions, but without your support to the ministry we would not be able to offer the quality of ministry that we do. No one will be turned away due to not being able to financially support the ministry as there are other ways to support our youth.

If this is the first child you are registering for any of the K-12th Programs, choose the "1st Child ($90)" option above. If this is your 2nd, 3rd, 4th, or 5th child being registered for the upcoming 2020-2021 year, please click the appropriate option above (i.e. If you are registering two children, the 1st child you will click "1st Child ($90)" option. The second registration form you fill out for your 2nd child you will choose the "2nd Child ($75)" option, etc.) 
If your teen is at least in 8th grade and preparing to celebrate the Sacrament of Confirmation this year, an additional form will be emailed at a later date that will include additional sacramental registration information.
